Agreement Waiver & Release Form

Before a student may try out/participate in any sport, including conditioning and practices, you must submit the Agreement Waiver & Release Form to the Athletic Director.

All student athletes are required to have an updated physical on record in order to participate in any tryouts, practices or games. Please ensure your child as an updated physical on file with the school before the start of their sports season.
